What we treat
What a chronic care visit includes.
Diabetes
A1C and glucose monitoring, medication management including GLP-1 and insulin when appropriate, and practical nutrition guidance you can follow.
High blood pressure
Home readings reviewed alongside office vitals, medication adjusted to your numbers and your side effects, and kidney and heart labs tracked.
Thyroid disorders
Hypothyroidism and hyperthyroidism monitored with regular labs, dosing adjusted to how you feel as well as what the numbers say.
Asthma
Controller and rescue inhaler plans, trigger review and follow-up so flare-ups become less frequent.
High cholesterol
Lipid panels, risk assessment and treatment that weighs medication against lifestyle changes with you in the decision.
Multiple conditions together
Many patients manage several of these at once. One provider coordinating all of them means fewer conflicting instructions.

Sugar Land and Fort Bend County
This visit in Sugar Land
The Sugar Land office gives Fort Bend County patients one provider for every ongoing condition. Diabetes, blood pressure, thyroid, asthma and cholesterol are followed at scheduled visits, with LabCorp labs drawn near home. Home readings and glucose logs can be reviewed by video between office visits, so the plan gets adjusted without an extra trip.
Cypress and Northwest Houston
This visit in Cypress
Patients along the 290 corridor manage chronic conditions at the Cypress office. Blood pressure, A1C, thyroid and lipid labs are ordered through LabCorp, and the results are reviewed with you in plain language. Routine follow-ups can move to video once care is established, with in-person visits when an exam changes the plan.

Questions
Questions about chronic care visits.
How often will I need to be seen?
It depends on the condition and how stable it is. Dorothy sets a follow-up interval with you at each visit and adjusts it as things change. Many follow-ups are done by video.
Can you take over management from my previous doctor?
Yes. Bring or request your records and recent labs so care continues without a gap in medication.
Is lab work included in the visit fee?
The $125 fee covers the visit. Labs are ordered through LabCorp and billed to your insurance, or at LabCorp's self-pay rate if you prefer.
Do you prescribe insulin and GLP-1 medications for diabetes?
Yes, when they are the right treatment for you. Medication choices are made together, considering your labs, your goals and what your pharmacy and insurance cover.
Can I manage my condition entirely by video?
Most ongoing management works well by video once care is established. An in-person visit is recommended for the first appointment and periodically for a physical exam.
