Book on Tebra
Tebra is our scheduling and patient portal. Choose Sugar Land, Cypress or a video visit, pick a time, and create your account. Intake and consent forms arrive by email; complete them before your visit.
- 2
Choose office or video
First visits are often best in person at the Sugar Land office or the Cypress office so vitals and an exam can be done. Video works well if you are established elsewhere and need a refill, a follow-up or a sick visit without an exam.

Check in
Arrive about 10 minutes early with your ID, medication list and insurance card. Payment for the visit is collected at check-in.

New patient questions
Before your first visit.
How do I book my first appointment?
Click any Book button to open our Tebra scheduling page, choose an office visit or video visit, and pick a time. You can also call either office.
What should I bring?
A photo ID, your current medication list or the bottles, your insurance card (used for labs, prescriptions and referrals), and any recent lab results or records you have. A list of questions helps too.
How early should I arrive?
About 10 minutes before your appointment. Intake forms are completed online after booking, so check-in is quick.
How much is the first visit?
$125 for most visits, or $175 if you need a controlled substance. Payment is collected at the visit by cash, card, HSA or FSA, and you receive an itemized receipt.
Will you use my insurance?
Not for the visit yet. Credentialing is in progress. Your insurance is used for LabCorp labs, prescriptions at your pharmacy, imaging and specialist referrals. See the insurance page.
What do I need for a first video visit?
A phone, tablet or computer with a camera and microphone, a private spot, and you must be physically located in Texas at the time of the visit. A link arrives before your appointment.
Can I transfer my records?
Yes. Ask your previous office to send records to us, or bring copies. Having your history at the first visit means care continues without a gap.
Your first visit starts with one booking.
